If you would like your shirt shipped prior to the meeting, we must receive your order by no later than March 29, 2013. Orders received from March 29th – April 26th will be available for pick-up at the Society Booth in Baltimore. |

On your order form, please indicate the amount you should be charged for tax. If you are having your shirt delivered to your home, and you reside in Illinois, Massachusetts, or Minnesota you will be charged the appropriate state sales tax indicated on the order form. If you are having your shirt delivered to your home and do not live in Illinois, Massachusetts, or Minnesota you will not be charged any sales tax.
If you want to pick up your shirt at the SLEEP 2013 Annual Meeting in Baltimore, you will be charged the Maryland state sales tax which is also indicated on the order form.
